Private letter or medical report request

Use this service to request a private letter or medical report.

Please ensure you check your email account including junk/spam for our response. Private letters and medical reports are completed at the discretion of the GP and requests may be rejected. All requests will be reviewed within 14 days of submission, and you will be informed of the associated fee which must be paid prior to completion.
