Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
Please complete this in full only leaving sections blank that do not apply to yourself.
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
We aim to process these forms within 5 working days. You will be contacted to provide us with photographic ID and proof of address via our secure portal after you have submitted the form. To make this process smoother, please prepare PDF copies of these documents ready to provide us when requested.
If you do not have access to the internet, or have difficulty in completing the form online, please attend the practice and our reception team will be able to assist in registering. Please remember to bring your ID document and proof of address when attending.
We accept the following:
- Personal ID: Current passport, photocard driving licence, birth certificate etc.
- Address ID: Utility bill, bank statement or council tax etc. (with your name, address and dated within the last 6 months).
We encourage you during the registration process to sign up for our online services or register with the NHS App to allow you to book appointments, request medications and view results at your convenience.
